Job Description: Transplant Coordinators are critical members of organ transplant teams, serving as the primary point of contact for patients undergoing the transplant process. They play a multifaceted role, coordinating all aspects of care from the initial evaluation for transplant suitability through post-transplant follow-up. Their responsibilities span clinical, administrative, and supportive functions, ensuring patients and their families are well-informed and prepared for the transplantation journey.
Key Responsibilities: Coordinate patient evaluations to determine their suitability for organ transplantation.
Facilitate communication between patients, families, and the multidisciplinary transplant team.
Manage the organ matching and allocation process, working closely with local and national organ donation organizations.
Provide pre- and post-transplant education to patients and families about the surgical process, medication regimens, and lifestyle modifications.
Monitor patients’ post-transplant recovery and compliance with follow-up care and medication regimens.
Collaborate with healthcare providers to address any complications or concerns that arise during the transplant process.
Maintain accurate and up-to-date patient records, including documentation for the transplant waiting list and post-transplant follow-up.
Education Qualifications: Nursing degree or a degree in a related health field; a Master’s degree may be preferred for certain positions.
Current licensure as a Registered Nurse (RN) or relevant healthcare professional certification, depending on the role and location.
Specialized training or certification in organ transplantation may be required or preferred.
Experience in critical care, nephrology, hepatology, or a related field is highly desirable.
Strong organizational, communication, and interpersonal skills.

About the Role: Serve as a liaison between the transplant patients and the broader medical and surgical team, ensuring a seamless continuum of care.
Act as educators and advocates for organ donation and transplantation within the community and healthcare settings.
Participate in multidisciplinary team meetings to discuss patient care plans, transplant candidacy, and post-transplant management.
Contribute to quality improvement initiatives aimed at enhancing the efficiency and effectiveness of the transplant process.
Engage in research activities related to organ transplantation, patient outcomes, and improvement strategies.
