Job Description
- Coordinating with other medical staff members, social workers, nurses, occupational therapists, physicians, and other healthcare providers to ensure that patients are receiving high quality care
- Assisting patients with activities of daily living such as bathing, dressing, grooming, eating, and hygiene
- Providing emotional support to patients and their families during difficult periods in the recovery process
- Facilitating patient discharge planning by creating a detailed plan that outlines the patient’s goals, needs, and preferences
- Ensuring that the facility complies with all federal, state, and local regulations regarding patient care
