Job Description

1. Coordinating with other medical staff members, social workers, nurses, occupational therapists, physicians, and other healthcare providers to ensure that patients are receiving high quality care
2. Assisting patients with activities of daily living such as bathing, dressing, grooming, eating, and hygiene
3. Providing emotional support to patients and their families during difficult periods in the recovery process
4. Facilitating patient discharge planning by creating a detailed plan that outlines the patient’s goals, needs, and preferences
5. Ensuring that the facility complies with all federal, state, and local regulations regarding patient care
