Job Description: The Pharmacist – OPD (Outpatient Department) is responsible for managing and dispensing medications to patients in the outpatient setting. They provide medication counseling, ensure the accuracy of prescriptions, and collaborate with healthcare providers to optimize patient care and therapeutic outcomes.
Key Responsibilities: Medication Dispensing: Accurately dispense medications prescribed by physicians, ensuring proper labeling, dosage, and instructions.
Patient Counseling: Provide comprehensive counseling to patients on medication usage, potential side effects, drug interactions, and storage requirements.
Prescription Verification: Review and verify prescriptions for accuracy, appropriateness, and potential drug interactions, resolving any discrepancies with prescribers.
Medication Therapy Management: Conduct medication therapy management (MTM) services, assessing patients’ medication regimens and making recommendations for optimization.
Drug Information: Serve as a resource for drug information, educating patients, caregivers, and healthcare providers on medications, new therapies, and treatment guidelines.
Inventory Management: Manage pharmacy inventory, ensuring the availability of essential medications, monitoring stock levels, and overseeing the procurement process.
Regulatory Compliance: Ensure compliance with all regulatory and legal requirements related to pharmacy practice, including record-keeping, reporting, and adherence to safety protocols.
Collaboration: Work closely with physicians, nurses, and other healthcare professionals to coordinate patient care and provide medication-related support.
Quality Assurance: Participate in quality assurance programs, medication error prevention initiatives, and continuous improvement efforts within the pharmacy.
Patient Safety: Implement and monitor patient safety measures, including adverse drug reaction reporting and medication reconciliation.
Clinical Services: Provide clinical services such as immunizations, health screenings, and chronic disease management programs.
Technology Utilization: Utilize pharmacy management software and electronic health records (EHR) to maintain accurate patient records and streamline pharmacy operations.
Professional Development: Engage in ongoing professional development and continuing education to stay current with advancements in pharmacotherapy and pharmacy practice.
Education and Training: Educate and train pharmacy interns, technicians, and other staff on best practices in pharmacy operations and patient care.
Customer Service: Deliver exceptional customer service, addressing patient inquiries and concerns with professionalism and empathy.
Education Qualifications: Doctor of Pharmacy (Pharm.D.) degree from an accredited institution.
Active pharmacist license in the state of practice.
Strong knowledge of pharmacology, therapeutics, and medication management.
