Job Description: The Staff Nurse in Home Care is a registered nurse (RN) responsible for providing skilled nursing care, assessments, treatments, and education to patients in their homes. They collaborate with interdisciplinary teams, coordinate care services, and promote patient independence and well-being.

Key Responsibilities: Patient Assessment and Care Planning:

Conduct comprehensive nursing assessments to evaluate patients’ health status, medical history, and care needs.
Develop individualized care plans, including medication management, wound care, IV therapy, and symptom management.
Collaborate with healthcare teams to coordinate patient care services and ensure continuity of care.
Skilled Nursing Interventions:

Perform skilled nursing procedures, such as medication administration, injections, catheter care, and dressing changes.
Monitor vital signs, assess for changes in patient condition, and report abnormalities to healthcare providers.
Implement nursing interventions to promote patient comfort, safety, and well-being.
Patient Education and Support:

Educate patients and caregivers about disease management, treatment plans, medication instructions, and self-care techniques.
Provide training on medical equipment use, home safety measures, and activities of daily living (ADLs).
Address patient questions, concerns, and provide emotional support and counseling as needed.
Documentation and Communication:

Maintain accurate and complete patient records, including nursing assessments, care plans, progress notes, and medication administration records.
Communicate with physicians, therapists, pharmacists, and other healthcare providers regarding patient progress, treatment updates, and care coordination.
Document patient education, interventions, and response to treatments.
Care Coordination and Collaboration:

Coordinate with home health aides, therapists, social workers, and other members of the healthcare team to ensure holistic patient care.
Participate in interdisciplinary care team meetings, case conferences, and care planning sessions.
Collaborate with community resources, support services, and discharge planning for patients transitioning from hospital to home care.

Education Qualifications: Registered Nurse (RN) licensure required; additional certifications in home health nursing or relevant specialties preferred.
Clinical experience in nursing, preferably in home care, community health, or acute care settings.
Knowledge of nursing protocols, standards of practice, and regulatory requirements in home care.
