Job Description
- Clinical Pharmacists are healthcare professionals specializing in optimizing medication use to promote health, wellness, and disease prevention.
- They work directly with patients and other healthcare providers to ensure that medications prescribed contribute to the best possible health outcomes.
- Clinical Pharmacists have specialized knowledge of medications and their mechanisms of action, side effects, interactions, and proper use.
Key Responsibilities
- Medication Management: Assess the appropriateness and effectiveness of a patient's medications to ensure optimal therapeutic outcomes.
- Patient Care: Provide direct patient care as part of a healthcare team, including medication therapy management, counseling, and education.
- Interdisciplinary Collaboration: Work closely with physicians, nurses, and other healthcare professionals to develop and implement patient care plans.
- Medication Safety: Monitor for potential drug interactions, adverse drug reactions, and contraindications to minimize risks associated with medication therapy.
- Education and Advocacy: Educate patients on the proper use and potential side effects of their medications and advocate for their health and well-being.
- Clinical Research: Participate in clinical trials and research to advance the field of pharmacotherapy.
Education Qualifications
- Education: Doctor of Pharmacy (Pharm.D.) degree from an accredited pharmacy school.
- Licensure: Must be licensed to practice pharmacy in the state where they work, which typically requires passing the North American Pharmacist Licensure Examination (NAPLEX) and a state-specific law exam.
- Residency: Completion of a post-graduate year one (PGY1) residency is highly recommended, and a post-graduate year two (PGY2) residency may be required for certain specialties.
