Job Description

1. Explain Diabetes Clinic Package to the patient in brief.
2. Tracking the status of patients treatment progress.
3. Collaborates, advocates, and confers other members of the diabetes care team in developing patient centric diabetes plans.
4. Engages throughout the programm and serves as a role model of professionalism.
5. Integrates skills and knowledge for self-management of diabetes into clinical practice.
6. Participates in meetings, serves on committees, and represents the department and hospital/facility in community outreach efforts as appropriate.
7. Completes comprehensive assessments for each patient including emotional and behavioral health, interprets personal health data, develops an individualized care plan based on the patientassessed needs and goals and promotes successful self-management.
8. Active participation in improving quality of care and outcomes for those living with, at risk for, and affected by diabetes.
