Job Description: The Medical Social Worker in Home Care is a vital member of the healthcare team, specializing in providing psychosocial support, counseling, and advocacy services to patients and their families in the home care setting. They assess social and emotional factors impacting patient care, coordinate community resources, and facilitate access to support services.

Key Responsibilities: Conduct comprehensive psychosocial assessments to identify patient and family needs, strengths, and challenges.
Provide counseling, emotional support, and coping strategies to patients and families facing healthcare-related stressors.
Collaborate with interdisciplinary team members to develop care plans that address social and emotional needs.
Care Coordination and Advocacy:

Coordinate access to community resources, social services, financial assistance programs, and support groups.
Advocate for patients’ rights, preferences, and access to appropriate healthcare services.
Assist with healthcare decision-making, advance care planning, and end-of-life care discussions.
Patient Education and Support:

Educate patients and families about healthcare options, treatment plans, and available resources.
Provide guidance on navigating the healthcare system, including insurance coverage, benefits, and entitlements.
Facilitate communication between patients, families, healthcare providers, and community agencies.
Crisis Intervention and Risk Assessment:

Conduct crisis interventions and risk assessments for patients experiencing social or mental health crises.
Collaborate with mental health professionals, crisis intervention teams, and emergency services as needed.
Develop safety plans, referrals, and follow-up support for at-risk patients.
Documentation and Collaboration:

Maintain accurate and confidential documentation of social work assessments, interventions, and progress notes.
Collaborate with healthcare team members, including nurses, therapists, and physicians, to coordinate patient care.
Participate in interdisciplinary team meetings, case conferences, and care planning sessions.

Education Qualifications: Master’s degree in social work (MSW) from an accredited program; state licensure as a Clinical Social Worker (LCSW) or equivalent required.
Clinical experience in medical social work, preferably in home care, hospice, or healthcare settings.
Knowledge of psychosocial assessment, counseling techniques, crisis intervention, and community resources.
