Job Description: The Patient Care Coordinator is responsible for ensuring a smooth and efficient patient experience by facilitating communication between patients, healthcare providers, and support staff. This role involves managing patient appointments, coordinating care plans, and assisting patients through their treatment journeys. The Patient Care Coordinator helps ensure patients receive timely and appropriate medical care, enhancing their overall experience with the healthcare system. This position also supports patient education, discharge planning, and acts as a liaison to address any concerns or issues that arise throughout the care process.
Key Responsibilities: Patient Liaison: Serve as the primary point of contact for patients, ensuring they understand their treatment plans and the steps involved in their care. Provide support and answer questions about appointments, procedures, and healthcare processes.
Appointment Coordination: Schedule patient appointments, follow-up visits, and diagnostic tests in a timely manner. Ensure patients are informed of all relevant details and provide reminders to minimize missed appointments.
Care Plan Coordination: Work with doctors, nurses, and other healthcare providers to develop and implement care plans tailored to individual patient needs. Ensure all necessary treatments, tests, and medications are arranged and completed.
Patient Education: Educate patients about their medical conditions, treatment options, and post-care instructions. Provide materials and resources to help patients understand their treatment plans and health management strategies.
Administrative Support: Maintain accurate patient records, including medical history, treatment plans, appointment schedules, and follow-up notes. Ensure all documentation is up-to-date and compliant with healthcare regulations.
Patient Advocacy: Advocate for patients, addressing any concerns or obstacles they may encounter during their care. Work with healthcare providers to ensure patients receive the highest standard of care.
Insurance and Billing Support: Assist patients with understanding insurance coverage, billing procedures, and any financial concerns related to their care.
Follow-Up: Monitor patient progress through treatment and coordinate follow-up care, ensuring that patients receive any additional support or services required post-treatment.
Problem Resolution: Address patient complaints, concerns, and feedback, ensuring that any issues are resolved quickly and effectively to improve the patient experience.
Collaboration: Collaborate with medical teams, specialists, and departments to ensure seamless care delivery and coordination of services.
Education Qualifications: Bachelor’s degree in Healthcare Administration, Nursing, Social Work, or a related field is preferred.
Experience in a patient care or healthcare coordination role is highly desirable.
Strong knowledge of healthcare systems, patient care practices, and insurance processes.
Basic Life Support (BLS) certification is a plus.
Proficiency in electronic health records (EHR) and scheduling software.
Knowledge of HIPAA regulations and patient confidentiality protocols.
