Job Description: The Pharmacist – IPD (Inpatient Department) is integral to the hospital care team, responsible for managing and optimizing medication use for hospitalized patients. They ensure accurate medication dispensing, monitor therapeutic regimens, and collaborate with healthcare professionals to enhance patient outcomes in an inpatient setting.

Key Responsibilities: Medication Dispensing: Prepare and dispense medications accurately for inpatient use, adhering to physician orders and hospital protocols.
Patient Monitoring: Regularly review patient medication profiles, monitor for drug interactions, adverse effects, and efficacy, and make necessary adjustments.
Clinical Rounds: Participate in multidisciplinary clinical rounds, providing pharmacological insights and recommendations to optimize patient treatment plans.
Medication Reconciliation: Conduct thorough medication reconciliation upon patient admission, transfer, and discharge to prevent medication errors.
Drug Information: Serve as a primary resource for drug information, offering guidance to healthcare providers on medication choices, dosing, and administration.
Therapeutic Drug Monitoring: Perform therapeutic drug monitoring (TDM) for high-risk medications, adjusting dosages based on patient-specific factors.
Sterile Compounding: Prepare sterile and non-sterile compounded medications, including intravenous admixtures, ensuring adherence to safety and quality standards.
Inventory Management: Oversee the management of medication inventories, ensuring the availability of necessary drugs and maintaining proper storage conditions.
Patient Counseling: Provide counseling to patients and their families on discharge medications, ensuring understanding and compliance with therapeutic regimens.
Regulatory Compliance: Ensure compliance with hospital policies, state and federal regulations, and standards of practice, including proper documentation and reporting.
Quality Improvement: Participate in quality improvement initiatives, medication safety programs, and clinical audits to enhance pharmacy services and patient care.
Clinical Documentation: Maintain accurate and thorough documentation of clinical interventions, medication reviews, and patient outcomes in the hospital’s electronic health record (EHR) system.
Education and Training: Educate and train healthcare staff, including nurses and pharmacy technicians, on medication management and safety practices.
Antimicrobial Stewardship: Contribute to antimicrobial stewardship programs by monitoring antibiotic use and promoting appropriate prescribing practices.
Emergency Response: Respond to medical emergencies, such as code blue situations, providing critical medication support and expertise.

Education Qualifications: Doctor of Pharmacy (Pharm.D.) degree from an accredited institution.
Active pharmacist license in the state of practice.
Residency training (PGY1 and/or PGY2) in hospital pharmacy or clinical pharmacy preferred.
Strong clinical knowledge of pharmacotherapy and medication management.
Excellent communication and teamwork skills.
Proficiency in using electronic health records (EHR) and pharmacy management software.
